Laura-

I always find that the order in which you release keystrokes and the mouse
is confusing when transforming. You may want to let go of the mouse button
first, while still holding Ctrl-Alt-Shift. If that doesn't work, try it the
other way.

However, if you're using a placed object, I'd highly recommend using
Photoshop to transform if the object is a raster (pixel-based) image. You
can always re-place it back into Illustrator. If you can swing it, upgrade
to CS3 or CS4, where you can place the object as a Smart Object; that way,
any changes you make outside of the program are automatically updated.
