I'm not sure what you're talking about. Topic headings go in the TOC.
They don't directly affect the index, the author can add them or not
as appropriate.
If I didn't think it would help the reader, then I wouldn't define a
"Create" section for the index.
I don't believe that the delivery format has any effect on what
belongs in an index--except that when distribution is print only, it's
important to have a really good index, and if possible I hire a
professional indexer.
