Hey Y'all -

How do you survey your SME community to gather feedback on writer's
performance? Do you send a formal survey, use email, forms, etc.?

Do you use an internal email format; or, something like Survey Monkey to
gather feedback? What kinds of questions do you ask?

Any samples would be most appreciated.
