I would use 'add' to imply creating a new member of a series, 'create' to imply configuring or building something that is thought of stand-alone. Therefore I would 'add' a new user to an account, but 'create' a new document. I would 'add a radio button' to a widget panel (because it's one of a group), but I would 'create' a widget (because although it might be part of a library, I treat and think of the object individually).

'New' is redundant for the reasons others have stated.
