I'd choose: "very frustrated that you are trying to evaluate someone using multiple choice questions":-).

It doesn't really matter how good the questions are, it's a bogus situation.

If they are hiring, then they are looking for red flags (I'd call either of the "frustrated" answers red flags for a company that doesn't have the patience to do a sensible evaluation).

If the person is a current employee, then they are either going through the motions, and the survey will disappear into his/her file, or they are looking for outlier answers to hold over the employee, which to me again means the "frustrated" answers.

If they were trying to actually evaluate an individual employee or candidate, they would ask open-ended questions and read the responses, or even better, call you on the phone and have a conversation.

So, to answer the original question, I'd pick the "somewhat impatient" answer, unless you want to torpedo the evaluation, and make a note not to work for that company:-).
