I generally use sentence case if the text is formatted distinctly as a heading... if it sticks out like a sore thumb already I don't see any need to impose additional capitalization. I'm inclined to feel that it's a relic of the typewriter age. I know that flies in the face of both tradition and marketing standards and that I'm an outlier in this realm... I'm just a tad OCD about that sort of thing. Of course, I'll follow a client style guide when there actually is one and it specifies title case but most of the time for lack of an actual style guide, I become the default style guide.
