I'll add that you don't need these topics (chapters and sub-headings) to display in HTML now. But you never know. And the issue is not only HTML -- it's any structured, reusable content repository. Repeated names hamper the processing you can do on your source content. And again... You never know.
I have the same situation, and I do it this way:Foo -- Context 1Foo -- Context 2
