Jim, I agree with your engineer's point. It sounds like
a good idea to me.
I just picked up a book to look for an example, and this
book (Horton's _Designing and Writing Online Documentation_)
did not include the glossary entry in the index.
So, what do *I* think? I am not sure. Either choice looks
viable. I think I am leaning toward your first listed choice.
