Hello:

First I would like to thank the folks who responded
to my post in which I asked for suggestions on
feedback forms. Most of the e-mails I received were
from others who also want advice. Two kind-hearted
souls shared their forms with me and 1 person
suggested the book
Exploring Requirements: Quality Before Design
by Donald Gause and Gerald Weinberg.

Regarding my project, the user manual feedback
form is my baby and it is coming along quite nicely.
I am developing a mini-tutorial with occasional
questions. I have interviewed the users and they
are more than willing to fill out the forms.

The product mgr is developing the feedback
forms for the product and his consists of questions
like _Do you like the color yes/no_ He is only
concerned with identifying factors which would
prevent the user from buying the product.

Some tips I have learned about generating feedback:
Use graphics on the forms
Use humor, if possible
All questions do not have to be in the same format
For each question provide a list of possible answers, the more specific the
better
