My copy of CMS is at home, but, if I remember correctly...

Yes, the first word should be cap'd.
If one member of the list requires a period (to end a sentence),
then all members of the list should be terminated with a period
whether they are complete sentences or not. Otherwise, no
terminal punctuation is required.
