One should use italics in end-user works in exactly the same way book
publishers do. End-users are used to "real" books, published by "real"
publishers, using standards such as those put down by the University
of Chicago Press in the CHIGAGO MANUAL OF STYLE. There's no point
getting creative in the realm of basic typesetting.
