We use Visual Basic for this. Of course, the risk you run with this
approach is to make the thing appear "usable" and thus sometimes modify
the interface (at the user's expense) in order to deal with program
constraints.
I attended a presentation at CMU about some folks who had built such a
prototyping tool on top of HTML. You could probably go a long way with
tables and forms.
