I've produced booklets for CD cases, and have named them by
content. I describe them (in plans, schedules, etc.) as booklets
or inserts for CD jewelcases--communicating how the content will
be presented can be a big challenge!

Some names for these booklets that I have used include Installing
..., Getting Started with ..., and ... User Guide. I have also
left them unnamed, when I've printed information on the inside of
the cover insert (or made a folded, four-side cover insert). The
heading(s) for the information can serve as the title.

In the music industry, the booklets are known as 'liner notes.'
I don't see why this term can't extend to software booklets as
well -dot- TECHWR-L -at- LISTSERV -dot- OKSTATE -dot- EDU
