The punctuation goes inside or outside the quotation depending on
where it should go.
If the person being quoted 'said' the punctuation, the punctuation
goes inside. If the person quoting them added it, it gowes on the
outside.
For example:
Iain wrote "I really struggle with understanding why people find this
difficult."
Iain wrote "I really struggle with understanding why", and displayed a
measure of irritation that the subject did not deserve. It was early
morning in his timezone!
