I think the quotation mark looks a bit awkward hanging outside the
period or comma, too.

In what little print work I do nowadays, I "stack" the period or comma
and the quotation mark, so that each is placed exactly as it would be if
the other were absent. It takes a little extra effort, but it looks
much better to me than either of these alternatives:

Type "test." /* If the period were *supposed* to be typed,
Type "test". /* I'd set it as in the first example.

I've never heard a single complaint from a client or reader about the
stacked punctuation. It makes me wonder why none of the "expert" font
sets seem to include such a ligature.
