If you go with the "How to. . ." construction, however, your automatically generated table of contents might a little less than absolutely user friendly, with several chapter or sub-heading beginning with "How to . . . ."

If they really insist on no gerunds, you could try going with the question
format:

How Do I Add a Heading?

That would eliminate the concern that it is "required" and fulfill the no
gerund rule at the same time.
Just a thought.
