Aha -- I get to give back, because I asked that very question about a year
ago.

In Acrobat Exchange, use the text selection tool to select the text you
want bookmarked, and click Ctrl-B. That will make a bookmark in the left
column. Once you have the bookmark, you can type in the name of it (in
theory the selected text will become the name, but that seldom works for
me), and you can promote and demote bookmark levels.
