For a general tome, there's William Hortons's "Designing & Writing
Online Documentation."

For WinHelp specific issues, including design and technical information,
the 2 top books I know of are Boggan, Farkas, and Welinske's "Developing
Online Help for Windows 95" and Deaton and Lockett Zubak's "Designing
Windows 95 Help."

The only book I know of (so far) on HTML Help is Steve Wexler's "HTML
Help Authoring Kit."
