I have read a lot of software development and documentation related
literature. I have found only one work that clearly and concisely explains the required processes to an eight grade level.

I suggest reading the first few chapters of the following (contains plenty of examples):

Structured Systems Analysis and Specification by Tom DeMarco, 1979.
Available through Yourdon Press.

I'm looking for an example of a proposal to document software to share with my undergraduate students. Tese are students who have no experience with documentation or proposals.
