Hi,

Once again I can only suggest you look at AuthorIT. It produces 100% pure
HTML with 100% pure HTML TOC and Indexes. Look at the examples on their
site http://www.author-it.com/examples.mv.

Unfortunately, this is just the way it is. This is one of the several
reasons that CHM files are not recommended for Web-based online help.
