Acronyms--How often do you spell them out?

I just worked on acronyms for my government document earlier today
(175 for a 256-page document). I'm going to be going back through the
document to make sure that only the first instance of each one is
spelled out. The document is a mish-mash of content from different
engineers, and so things are very inconsistent right now.
So, first time only is the rule with the DoN document I'm working on.
