Hi Michael,
We don't use them ourselves - yet! - but if I were tasked to create a
form for recording data (in general, not specifically perf appraisals)
I would rather:
- set up a web-server (intranet-only, not visible on the Internet
unless the company had offices at multiple locations)
- use a scripting language like PHP or ASP to gather details submitted
in the form and store them in a database
- perhaps, add more functionality, like routing a filled form to a
supervisor or higher, for approval/review, or maybe use the collected
data to provide management reports (how many people are happy... how
many feel they have been under-rewarded for the efforts they have put
in.. etc.)
Now that's because I've been a programmer for several years, and I
understand that not everyone likes the coding approach to solving
problems.
Do you have any constraint or compulsion to use Word for your forms?
