This is a great suggestion, Joel. This is the most common cause of
problems with STYLEREF fields. STYLEREF picks up the first instance
of the style on the current page, or the last preceding instance if it
the style is not present on the current page. Having a second, empty
instance of the style would produce exactly the behavior Zen describes
(the field working properly on the first page but not on subsequent
pages).
Use Find to search for every instance of the the specific style name
in the document (you'll find Style under the Formatting menu when
you click the More button in the Find/Replace dialog) and make sure
that only the actual chapter title is assigned to the Heading 1 style.
