FrameMaker problem with table numbering

The counter is clearly somehow being reset to zero (and then your
counter increments it to 1 each time). So I'd look for a paragraph
format that appears to be resetting the counter to zero. It will include
in its autonumber format something like <n=0> or <=0>. How such a change
could have been introduced by the actions you describe I can't imagine,
but if I saw that behaviour without knowing the history of the document,
that's what I'd assume was happening.

It may be worth bearing in mind that a counter can exist on a paragraph
format that does not actually display text, and which will therefore not
stand out as a potential candidate for checking. For example, in the
template I use, the Graphic paragraph format has a counter which
increments the figure number, but no number is actually displayed there.
The Figure:Title paragraph format that always follows the Graphic format
displays the number -- its autonumber format includes an "n" which
outputs the latest value of n as incremented by the Graphic format. (I'm
not sure why it's done this way -- someone else designed the template.)

If you can't find where the resetting to zero is happening, try altering
the numbering format for table titles as follows:

T:Table <$chapnum>.< >< >< >< >< ><n+>

My admittedly limited understanding of FrameMaker's autonumbering
suggests this should make your table title autonumbering independent of
any other paragraph format that might be presently resetting it to zero.
