I'll second the recommendation of the indexing chapter in
"Read Me First". It's really a good introduction. And about
a half-dozen years ago, when our pubs group was *much*
larger, we contracted with Lori Lathrop, a prominent
professional indexer, to run a 2-day indexing workshop,
and she also recommended that chapter.
