Avoiding documentation bottlenecks while maintaining quality

Process is like anything else. An intelligent and realistic one works
well, but an inspecific, unrealistic or downright stupid one fails in
any situation. Process alone can't solve the world's problems, because
quality of intelligence and dedication in employees is essential, but
if your people are OK then a good process can make all the difference
in the world.

It seems to me that "process" is a maligned word because so many people
misuse it, like they misuse the term "bacon burger" to mean some tiny
half-soybean quarter pounder that comes from a drive-thru.
