Vendor for laminated quick-reference?

Do you mean a vendor to print and laminate, or someone
to design the sheet itself?  Every printing vendor I've ever
used could laminate and punch (they do it commonly for
covers).  I'd say check your yellow pages, as for printing
local is almost always best.
