Hi Susan,
Some useful questions might be (not necessarily in order)...

1. What questions remained unanswered after the training?

2. What did you expect to be able to do back on site that you couldn't do?

3. What information did you remember most clearly?

They are all questions about learning OUTCOMES.

I have found that people can be disappointed in training outcomes but
answer general agenda, materials, trainer questions quite positively. When
I was with an international computer company, I had world-wide
responsibility for a development software. Training sessions all around the
world averaged 7.5/10 in training evaluations... but for courses that
customers constantly complained about. WE WEREN'T ASKING QUESTIONS ABOUT
OUTCOMES.
