Do you mean that your manager reported the omissions to her boss? Did your manager completely omit the fact that she told you to leave it out? Did you explain the omissions and the circumstances to the person who sent this letter?

I think you can, at very least, respond in writing to the documentation. Make note of all that you tried to do to avert the situation. If your manager actually stepped aside to let you take the shot, then I'd be very concerned about her. In any case, the most you can do is make sure to document what you did and why, and include what advice you were given by your manager. Obviously, it won't hurt to watch for other job opportunities, but you might still be able to keep this position. If you can think of anything you DID do for which you are culpable, you should accept responsibility for it and explain how you will avoid the same errors in the future. It's important that you not focus on blaming others but on clarifying who did or said what. That includes anything you might have done that contributed to the situation. It might also help to include copies of the notes and specifications from which you were working, emails to and from the SMEs, and any other supporting documentation you can provide.
