I generally provide the menu commands in the main flow of the manual.

In a section on navigation talk about the general idea of keyboard
shortcuts/navigation (and how to find the specific keystroke shortcuts
available on each menu and screen). Buttons that have the same function on
many screens are also documented here (and not repeatedly throughout the
main flow, except when there is something special to note).

When documenting an application with toolbars, I usually provide both the
toolbar and the menu way of doing things in the main flow.
