Actually, Acrobat 5 has a Save As RTF selection that will extract most of the text in half-decent format. And if the document was created in Word, with the correct options selected when the PDF was created, the RTF allegedly will include style information, TOC tags, etc., as well. Can't promise anything about the layout, though.
