First of all, the meanings of the two sentences are different. Others have already commented on that. But what I'm wondering about is the use of the passive...."are included"

Why not:
"This form only includes factors that are the same across all pipelines and internal companies." or "This form includes only those factors that are the same across all pipelines and internal companies." (if you're describing the content of the form(a rewrite for your sentence))

or

"This is the only form that includes factors that are the same across all pipelines and internal companies." (if you're describing the fact that the mentioned factors appear on this form and nowhere else (a rewrite for the student's sentence))

Just a thought
