Hi,
I've come rather late to this discussion, but let me offer one
suggestion. I had a similar problem a couple of years ago with a
bookmarkless PDF. The solution we used was called Compose
(http://www.infodata.com/compose.asp), which, among other things,
enables you automagically generate bookmarks based on formatting styles
in your PDF. So, for example, you can create a rule that says "make a
2nd-level bookmark for every occurrence of 16-pt Arial Bold". It was a
bit buggy, but it saved us a lot of work. Frankly, at the time I thought
it was priced ridiculously high for a PDF tool, but it saved our butts.
