Caroline Jarrett has provided a number of topics on forms here. While it may
not be tailored exactly to what you are doing, you will probably still find
answers to a number of your questions here. If you don't she has an email

Does anyone have any resources for designing forms?

I am making forms in Word and then using Acrobat forms.
I'm going to get a book on Acrobat forms, but I'd also like some tips on how
to design the forms for optimum usability.
Ideally, the form would eliminate the need for procedures, in some cases.
