Use the PDFMaker macro in Word, and make sure it's set up to do what you want.

1. From the Acrobat menu, choose Change Conversion Settings.

2. On the Office tab, make sure the following options are enabled:

General: Convert Cross-Document Links

General: Convert Internet Links

Word Features: Cross-References & TOC Links

(I also enable Page Labels.)

3. On the Bookmarks tab, enable Convert Word Headings to Bookmarks.

4. Click on the OK button.

5. From the Acrobat menu, choose Convert to Adobe PDF.
