Entities only work in DocBook SGML, not XML. It's an XML issue, not a
DocBook issue. It's one of the compromises W3C made when they defined
XML. I made the same mistake when I read the Definitive Guide.

That's one of the reasons some folks still stick with SGML for DocBook.

I'm not sure what the similar concept is in XML, but if anyone knows
please let me know.
