The smoker would probably expect to receive the same
nominal benefits package as other employees. The smoker
could be seen to be deliberately choosing to raise the
cost of group insurance/bennies for that employer,
and indirectly for the other employees. If the loss of
that particular candidate were considered less than the
anticipated longterm losses due to having him/her in
the insuree pool, then that looks like a good business
decision on the part of the employer. No ethical consideration
there.

The smoker might not LIKE that his/her employment chances
ultimately hinged on her/his choice of bad habits (or of
admitting to that choice of bad habits...), but the bare
fact that the employer made that choice is not an ethical
problem on the part of the employer. If the employer had
legitimate reason to believe that it would be a net cost
to the company to hire that prospect, rather than another,
then it would be unethical for the employer to pick the
smoker (after all, they have responsibility to their shareholders
and to their other employees).
