Evid,
Based on the information at hand, it sounds like the document is not
connecting to the style sheet. This usually occurs with relative URLs and
a new document in a folder. Try an absolute URL in the document to see if
corrects the problem. (If this corrects the problem, you will need to
experiment with trimming the root folder from the absolute URL, confirming
that things still work, and then make the trimmed URL the standard for
your template.)
