There doesn't seem to be a universal definition - everybody is giving you
their own personal definitions. In that case, for the purposes of the
questionnaire why not just answer "Yes - I am a published author?" If
anybody challenges you, show them your portfolio. If they say "That's not
published," you can say "It most certainly is."
