We use lots of text insets in several documents, some of which have index markers in the text insets. They all seem to work just fine. We used to have two chapters that were nothing but one big text inset. All of the indexing was within the text inset and it worked just fine.
We use the native Frame index marker and IXGen to help out.
One thing I can think of to check is to make sure that for some reason the index markers in the text insets haven't been changed to some other marker type.
We use Web Works Publisher to produce help files from our Frame files and occassionally an index marker suddently ends up as a Filename or TopicAlias marker. You might do a quick check for that kind of happening. But other than that, the standard Frame marker seem to work just fine.

Details: Frame 7.1, Windows 2000, "regular" Frame (rather than structured).
I've inserted index markers into a text inset that is used several times
within a multi-chapter book. When I save all files and update the 'dex, only
the first occurrence of the text inset is populated to the index.
I'd initially been using Silicon Prairie Software's Index Tools Pro (which
worked fine from the text inset/index marker) perspective, but there were so
many other problems with it that I scrubbed the 'dex in progress, reloaded
the files, and am rebuilding the index using Frame's native indexing tool.
Is there a way to use index markers in text insets? Fast responses are
appreciated - I'm to provide a draft of the index by mid-day Friday.
