Our I.S. gang wrote an app that allows me to bookmark and combine PDFs by manipulating XML that feeds into the app. I can also add a password to protect the combined PDF.
I don't think Adobe allows you to combine password-protected PDFs - at least not in version 6.0. I tried doing that yesterday and hit a wall.
