I'm using Word 2000. But I can't find a Reference > Caption from the Insert menu.

As far as I can tell, just making a style named Table Caption
doesn't cut it. You have to go to the table and then select
Insert > Reference > Caption to get a real caption. When I
did this, the caption did show up in the cross-reference db.
