In each of the volumes. If a reader has only one volume available to them (e.g., they borrowed it from the library) and that book doesn't happen to be the one with the index, how do they find things? Although you could put only a volume-specific index in each volume, it's often very useful to have a master index that tells the user clearly "you can't find what you're looking for in this volume, but can find it in Volume 3".

On the other hand, you might not be able to sell your boss on the extra cost of repeating the index multiple times. If that's the case, create a single index for each volume, and make the overall index available as part of the online help. No cost to put it online, unlike printing it.

Ditto. That's particularly true for online information, since it costs nothing to repeat the TOC as many times as you want.
