Headings and ideal template for navigable user guides

The discussion on headings takes us to the next question. What makes for an
optimum and navigable template? Let's talk about FrameMaker in particular.
Users should be able to find information intuitively, quickly, and with
minimum fuss.
What do people do? Create their own, use their company's standard ones, or
buy customized versions from experts? What are the issues with Structured
FrameMaker? What seems to work better there for XML output? I will compile
answers and if anybody has non-copyrighted home-grown templates, we can post
them for everyone's benefit.
