Lou, have you tested OpenOffice.org Writer? I know it imports Word docs
cleanly and I know it exports PDF, but I don't know if it preserves the TOC
bookmarks.

This page implies that it was (or it was intended to be) added in 2.0,
which is now out:

Just to note that working links in PDF documents is coming in OOo 2.0,
due H1 2005. For more new features see http://specs.openoffice.org/

Anyone who needs improved PDF creation with the current version of
OOo should look at extendedPDF from JDiSoftware. This is a macro that
adds rich markup to PDF, including bookmarks and hyperlinks.
I have used it with great success on a range of documents both
large and small.
