HTML Help Workshop has been mentioned along with several RoboHelp topics.
Here is a page on the Microsoft Web site about the newest version:

It provides a link to scroll through many programs to download the Help
workshop.

Evidently, this is part of the Office 2003 Resource Kit (ORK). I know
several people mentioned that use this to compile HTML Help with "embedded"
PDFs.  DISCLAIMER: I haven't tried this, so I don't know first-hand, but
some folks say it works.  After I've tried it, I will testify :-)
