Back on July 11, I asked for ideas for creating personnel evaluation forms using Word's Form Tool. Not one person replied, so I will try to make the request more general in nature.
Does anyone use performance appraisal forms that are completed online? If so, how were they created? What is a relatively simple, cost-effective way to do it?
