Technically, this is possible in PDFs through the use of form fields instead of links.
[Form fields -- in addition to launching actions -- can display tool tips, including graphics, when the active area is entered; see http://www.microtype.com/showcase/FormAsst/ButtonBar.pdf for an example]

However, I don't know of a tool which will help you transform the links to the required form fields; doing it manually is labor-intensive.
