If your plan-approved document suddenly grows by 12
topics because of developer feature creep, the impact on
documentation (schedule slip or reduced level of detail in other topics if schedule is inflexible) is recorded in plan
updates and agreed to by product owners. Otherwise the
new features go undocumented because they are not in
plan.

to be constantly reminded. Yeah, you could measure that.
