Tools: Problem with index page ranges using InDesign CS

Using InDesign CS on my Mac, and trying to index a book. On the
whole, it's going well, except for one vexing problem: When I insert
an index entry with a page range using the "To next use of
Style" (followed by choosing a style name), often all I get is only
the first page of the range. Sometimes it does work, and provides the
correct page range, but I can't see any difference in the Page
Reference Options dialogue box or in the Indexing Palette that would
explain the difference or hint at a solution.
For the moment, I'm kludging my way around the problem by using the
"for the next X pages" option, but I'd rather use the "to next use of
style" instead; that way, if I expand part of a chapter, the index
entry will update automatically.
Any thoughts on what's going wrong and how to fix it?
